Birth & Death Certificates

Birth or death certificates may be obtained either through SJBPH or directly through the Colorado Department of Public Health & Environment.

Birth and death certificates are available in the Durango and Pagosa Springs offices. For the Durango office, certificates will be ready the next business day at 10 a.m.

CERTIFICATE SERVICES

To obtain a birth certificate through SJBPH, you can submit an application in person at our office (you must present the required identification) or you can mail, fax, or email your application with a photocopy of your identification to certificates@sjbpublichealth.org and pay online on our website.

Birth certificates cost  $17.75 for the first copy and $10 for each additional copy. Effective July 1, 2018, the Office of the State Registrar of Vital Statistics will be increasing the birth certificate fee to $20.00 and any additional certificates will cost $13.00.

Birth certificates are confidential and can only be given to family members who can prove relationship or legal guardians/representatives.

To obtain a death certificate through SJBPH, you can submit an application in person at our office (you must present the required identification) or you can mail, fax, or email your application with a photocopy of your identification to certificates@sjbpublichealth.org and pay online on our website.

Death certificates cost $20.00 for the first copy and $13 for each additional copy.

Death certificates are confidential and can only be given to family members and people who may demonstrate a direct and tangible interest.
