San Juan Basin Public Health is hiring a Safecare Provider.  Safecare is an internationally recognized, evidence-based, in-home parent support program that provides direct skills training to parents and caregivers. This position will deliver the program in a home setting.  In addition to home visits the Safecare provider will recruit and retain clients and carry a caseload, attend required training, and establish ongoing relationships with community partners in collaboration with the Safecare Supervisor. A High School Diploma or GED and one year of office or health clinic support activities, or relevant experience is required.  The pay range for this position is $14.91 – $21.63 per hour plus benefits.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.  Position is scheduled to close 7/16/18, or until an adequate pool is reached.

Safecare Provider

San Juan Basin Public Health is hiring a Safecare Supervisor.  Safecare is an internationally recognized, evidence-based, in-home parent support program that provides direct skills training to parents and caregivers. This position will be 50% Safecare Supervisor and 50% Safecare Provider. The supervisor will provide leadership for the Safecare provider(s) through education, problem solving, technical assistance, and joint visits.  An Associate’s degree and two years of healthcare, public health or related experience is required.  A Bachelor’s degree is preferred.  The pay range for this position is $21.04 – $30.52 per hour plus benefits.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.  Position is scheduled to close 7/16/18, or until an adequate pool is reached.

Safecare Supervisor

Job Description

San Juan Basin Public Health’s Single Entry Point department is hiring a Programs Case Manager. This position provides ongoing case management services to elderly and disabled individuals within La Plata County.   Duties include conducting functional assessments, developing individualized service plans, nursing facility assessments, monitoring client’s services and needs, adapting services to meet changing needs of clients, and entering pre-authorization requests in the Medicaid system for providers to billing.  Tasks also include entering data in the state database in a timely manner and overseeing the utilization and quality of services.    The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients.  A bachelor’s degree in behavioral science or related filed is required.  The pay range for this position is $19.24 – $27.90 per hour plus benefits.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.  Position is scheduled to close 7/20/18, or until an adequate pool is reached.

San Juan Basin Public Health is hiring a Single Entry Point Program Manager.  This position will lead administration and management of the Single Entry Point (Options for Long-Term Care) Program and related case management services for elderly and disabled individuals. The ideal candidate will have knowledge and experience in case management, quality assurance, budgeting and staff supervision. The Program Manager will supervise a team of Case Managers and maintain a small caseload of clients. Ability to work as part of a team in a fast-paced environment is essential. Environmental Health Technician

San Juan Basin Public Health is hiring a part time Public Health Nurse Home Visitor.  This position will provide home visits to women and their families eligible for the Nurse Family Partnership (NFP) program. Nurses adhere to the NFP model of home visitation and nursing process guidelines, develop therapeutic relationships utilizing concepts of reflection and motivational interviewing, assess physical, emotional, social and environmental needs, goals and outcomes of clients relative the NFP program, and follow the nursing process in implementing NFP into Nursing practice.  Applicant must be a Registered Nurse in the state of Colorado.  BSN required.  Minimum of two years experience in maternal/child health or public health.  Must have CPR certification.  This position pays $24.51 to $36.77 per hour.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.
