San Juan Basin Public Health is hiring a Programs Case Manager.  This position provides case management services to elderly and disabled individuals within La Plata and San Juan Counties.  Case Manager is responsible for conducting functional assessments, developing individualized service plans, nursing facility assessments, monitoring client’s services and needs, adapting services to meet changing needs of clients through ongoing case management.   In addition, the Programs Case Manager must input client information in the state database in a timely manner.  The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients.   A Bachelor’s degree in Behavioral Science is required. The wage for this position is $19.24-$27.90, plus benefits.   Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. This position is scheduled to close 1/12/18, or until an adequate pool is reached.

Programs Case Manager

San Juan Basin Public Health is hiring a Family Health Care Coordinator.  This position performs intensive case management for multiple programs, with a focus on families & pregnant women in our region. The care coordinator will work with clients, community and agency partners to create clear service care plans, promote timely access to care, work to increase utilization of preventative care services. Knowledge of Medicaid, community resources, health programs & critical thinking helpful.  A Bachelor’s degree is preferred. The wage for this position is $16.36-$23.72, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. Family Health Care Coordinator

Job Description

San Juan Basin Public Health is hiring a Communities That Care Coordinator. This full-time position is responsible for overseeing a local community coalition aimed at preventing youth problem behaviors across La Plata County. The CTC Coordinator is tasked with using effective group facilitation skills to help guide the coalition through the CTC planning and implementation process. Duties also include facilitating meetings, recruiting and engaging key leaders and stakeholders from diverse sectors, data collection, assisting in writing grant proposals and reports, collaborating with partners and stakeholders to provide timely education, and communication regarding program initiatives. The applicant must have excellent communication skills, familiarity with local community, group facilitation skills, demonstrated ability to speak effectively and professionally in public to a variety of audiences, and demonstrated ability to form relationships with strategic partners. This position requires an Associate’s degree and two years of healthcare, public health or related experience.  Bachelor’s degree is preferred. The wage for this position is $17.80-$25.81, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. San Juan Basin Public Health is hiring an Administrative Assistant/Case Manager.  This part time position provides administrative support to case managers orchestrating services for the elderly and disable individuals within Archuleta County.  Duties also include preparing, organizing and maintaining files, responding to inquiries, referring clients to resources, conducting functional assessments for clients enter a nursing facility, as well as creating individualized service plan, and monitoring client’s services.  Additional duties include; entering client information in the state database in a timely manner.  The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients.  A bachelor’s degree in behavioral science is required.  The wage for this position is $19.24-$27.90 an hour plus benefits.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.  This position is scheduled to close 12/18/2017, or until an adequate pool is reached.
