San Juan Basin Public Health is hiring a Family Health Care Coordinator.  The Family Health Care Coordinator helps clients navigate the Medicaid/CHP+ systems, as well as non-Medicaid community systems.  This includes outreach serving La Plata, Archuleta, San Juan, Montezuma and Dolores County citizens who would like to access public programs and those who are already eligible for public programs, for those under the age of 21 or pregnant.  Through Health First Colorado’s (Medicaid) we connect families and pregnant women with resources to obtain treatment for medical, dental, vision, mental health, and developmental problems such as well-child visits, diagnosis and treatment services.  This position is scheduled to close 9/20/2019, or until an adequate pool is reached. Family Health Care Coordinator

Job Description

San Juan Basin Public Health is hiring a PRN Registered Nurse with experience providing health care services via tele-medicine.  This position will specifically help develop systems and coordinate clinics with medical providers in the San Juan Basin catchment area with the goal of providing tele-medicine for the pediatric population with special health care needs.  The qualifications for this position are licensure as a Registered Nurse in the state of Colorado and one year nursing experience, preferably with pediatrics, and experience with tele-health.  This position is scheduled to close 9/6/2019, or until an adequate pool is reached. San Juan Basin Public Health is hiring a part-time (26 hrs/wk) Clinic Outreach Coordinator.  This staff will assist in increasing breast, cervical, and colorectal cancer screening rates among Colorado women by conducting targeted outreach, organizing and participating in community events, providing education, and working directly with clients on barrier reduction.  This position is scheduled to close 9/16/19, or until an adequate pool is reached. Clinic Outreach Coordinator

Job Description

San Juan Basin Public Health is hiring a Director of Clinical and Emergency Preparedness Services. This position oversees clinical services (focusing on sexual health), Communicable Disease and emergency preparedness and response (EPR) activities at SJBPH. The Director serves as the subject matter expert regarding programs of the Division, seeks resources to support assigned programs, and coordinates with external agencies. Successful applicants will demonstrate experience managing staff and budgets as well as exceptional critical thinking skills. Knowledge of healthcare clinic management, Epidemiology and EPR principles are essential.  The Qualifications for this position are a bachelor’s degree in a related field, clinical licensure, and seven years of experience related to area of assignment including two years of supervisory experience is required.  This position is scheduled to close 9/13/19, or until an adequate pool is reached. San Juan Basin Public Health is hiring a Regional Epidemiologist. This position: Serves as the Southwest Regional Epidemiologist; Conducts and assists with timely and complete case, outbreak, and incident investigations as well as implementing disease control measures; Provides support in the development of a collaborative statewide network providing emergency preparedness and response assistance; Provides care coordination for clients referred for latent and active TB treatment. A Bachelor’s degree in nursing, social work, human services, public health, environmental science, or related field and five years of human services or public health programs related experience is required.  This position is scheduled to close July 15th, 2019, or until an adequate pool is reached.  The hiring range for this position is $25.92 to $38.89.  New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.
