Birth & Death Certificates
Colorado birth or death certificates may be obtained either through SJBPH or directly through the Colorado Department of Public Health and Environment.
To obtain a birth certificate through SJBPH, you can submit an application in person at our office (you must present the required identification) or you can mail, fax, or email your application with a photocopy of your identification and pay online on our website.
The birth certificate fee is $20 and $13 for any additional certificates.
San Juan Basin Public Health prints all birth certificates next business day. Certificates are not available at time of order.
Birth certificates are confidential and can only be given to family members who can prove a relationship or legal guardians/representatives.
Application
To obtain a death certificate through SJBPH, you can submit an application in person at our office (you must present the required identification) or you can mail, fax, or email your application with a photocopy of your identification and pay online on our website.
Death certificates cost $20 for the first copy and $13 for each additional copy.
San Juan Basin Public Health prints all death certificates the next business day when possible. Some death certificates (depending on date of death) require 5-7 business days to be released from the state for printing. Certificates for deaths that occurred in or before 1975 can only be printed in the county of death. Certificates are not available at time of order.
Death certificates are confidential and can only be given to family members and people who may demonstrate a direct and tangible interest.
