Care Coordination
The Care Coordination program of SJBPH is a function that supports information-sharing across providers, patients, and community organizations. Care Coordination is client-centered, and we support our community members with resources, referrals, and information and best serve those who have multiple needs that cannot be met by a single provider or organization.
The Children and Youth with Special Health Care Needs (HCP) nurse provide free services for clients and their families who live in La Plata and Archuleta counties. These children, aged 0 to 21, have needs beyond what is typical, for physical, developmental, behavioral, or emotional conditions.
The nurse care coordinator connects families with information and resources needed to manage their child's health care. This may include access to specialty care, planning for life transitions, communicating across the health care team, and finding local resources and support. Care coordination can look like information and a referral, or be more in-depth assistance in developing a plan of care that identifies and prioritizes the needs of the family.

The SJBPH Parents Plus Nurse assists La Plata County residents with parenting, health needs, health education, access to health and other related resources, and preventive care. Parents Plus is a nurse case management program that utilizes nursing assessment skills and support in a variety of settings. Parents Plus Project is part of an interdisciplinary team led by La Plata County Department of Human Services Temporary Assistance for Needy Families (TANF) Program.

SJBPH offers care coordination to assist community members with Medicaid and other benefits applications, connecting them to resources, finding medical or dental homes, and assisting with other identified needs.
