Community Health Assessment

Every five years, San Juan Basin Public Health takes a close look at the health and wellness needs of Archuleta and La Plata counties through a community health assessment. This assessment seeks input from community members to identify the top health and wellness priorities among our residents. The information that we gather through this assessment will inform a public health improvement plan that aims to address the priorities you tell us are important. Your health matters and so does your voice.

Both Archuleta and La Plata counties are in the process of forming individual health departments to launch on January 1, 2024, so this community health assessment will be more important than ever. The input you provide will help the counties identify public and environmental health priorities that will inform their paths forward.

This anonymous survey will take approximately 15 minutes to complete.
