San Juan Basin Public Health is hiring a full-time Public Health Nurse Home Visitor.  Strong families start with a great nurse.  Your guidance helps mothers succeed.  This position will provide home visits to women and their families eligible for the Nurse-Family Partnership (NFP) program.  More information on NFP is available at www.nursefamilypartnership.org.   Nurses adhere to the NFP model of home visitation and follow nursing process guidelines; develop therapeutic relationships; and assess physical, emotional, social, environmental needs and goals.  The relationship between nurse and mother is client-centered and strengths-based.  applicant must be a Registered Nurse in the state of Colorado, BSN required.  Experience in obstetrics, maternal and child health, community health or public health is encouraged.  Must have CPR certification.  This position is scheduled to close 06/21/19, or until an adequate pool is reached. The hiring range for this position is $29.10 to $36.38. New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.

Public Health Nurse Home Visitor

San Juan Basin Public Health is hiring a Public Health Nurse.  This position serves as a resource for pediatrics, and community health care providers by providing care coordination assistance, resources, and assist with Pediatric Neurology and Rehabilitation clinics in the area. Experience navigating pediatric health care systems is preferred, but not required.  This position is part time (30 hrs per week), with benefits. Licensure as a Registered Nurse in the state of Colorado required.  Position is scheduled to close 6/10/2019, or until an adequate pool is reached.  The hiring wage range for this position is $24.51 to $30.64.  New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.

San Juan Basin Public Health is requesting proposals from qualified candidates to consult on social media strategy concepts and social media best practice for organizations similar to SJBPH. Additionally, the chosen consultant will create a Social Media Strategic Plan for the agency, train staff on social media strategy, and implement aspects of the Social Media Strategic Plan. The project time frame is 6 months. Proposals will be accepted until May 31st at 5pm, or until an adequate pool has been reached. Social Media Strategy Goals:

  1. Gain a better understanding of the strengths of, and gaps in the agency’s existing social media channels, website, and e-mail marketing products.
  2. Increase traffic and engagement on all channels (including ones not yet utilized) to better serve as a go-to resource and subject matter expert for residents in SJBPH’s service area.
  3. Increase SJBPH communications staff expertise in social media strategy and tactics

Project Deliverables:

  1. Perform an audit of existing social media channels, website, and e-mail marketing products and provide recommendations based on audit
  2. Create an agency Social Media Strategic Plan
  3. Initiate implementation of Social Media Strategic Plan to increase traffic and engagement
  4. Train staff on strategy and tactics identified in Social Media Strategic Plan
  5. Launch Instagram page
  6. Track and monitor social media results to demonstrate return on investment

San Juan Basin Public Health is hiring a Health Equity Coordinator to implement the health equity work of the Equity in Emergency Preparedness (EEP) project, including overseeing the implementation of project activities and coordination with external partner agencies. EEP will ensure that all people living in Archuleta and La Plata counties have equitable opportunities to prepare for and respond to health events, natural disasters, and man-made emergencies, irrespective of socio-economic attainment, primary language spoken, or disability status. Collaborating with traditionally underrepresented and under-served populations to create resilient, connected communities will support population health and prevent future exacerbation of poor health outcomes stemming from emergencies.  Bilingual English-Spanish and an associate’s degree and two years of healthcare, public health, or related experience is preferred.  The pay range for this position is $17.80 to $25.81 an hour plus benefits.  Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. Position is scheduled to close 5/17/2019, or until an adequate pool is reached.
