San Juan Basin Public Health (SJBPH) is soliciting proposals from qualified individuals or organizations to create a website for the La Plata County Suicide Prevention Collaborative.  The intent of this project is to build a highly visible site that provides for the on-going communications needs of the collaborative which includes suicide prevention education, resources and event information.

Consultant will:

  • Work with SJBPH and existing partners to develop a website utilizing a platform selected by the contractor using the current collaborative graphics plan, branding and content.
  • Build basic site that is scalable for multiple browsers and devices which is easy to navigate and ADA accessible.  Written and visual content, including images and branded video, are easily distributed on platforms like Facebook, Twitter, Pinterest, and LinkedIn with the addition of social sharing buttons on website.
  • Links information requests to contact sheet shared by collaborative partners.

Train SJBPH staff and selected partners on the management of the website platform once site is built. Partners will access through secured portal.

San Juan Basin Public Health is hiring a Substance Use Prevention Coordinator. This position is responsible for performing community-wide outreach and program promotion in support of contracts.  Successful applicants will have experience in organizing community members involvement in local public health initiatives for youth such as substance misuse prevention, tobacco prevention, suicide prevention or other behavioral health/healthy behavior needs. This position collaborates with a diverse set of partners and stakeholders to provide timely education and communication regarding program initiatives; providing trainings and technical assistance to community stakeholders to build their capacity to engage equitably in public health initiatives; provide education directly to youth and caregivers to support building healthy communities; supporting required monthly reporting and program evaluation for funders and internal needs;  utilizing data collection tools and methodologies for program analysis; evaluation and monitoring program data and risk indicators to assess conditions; and assisting in writing grant proposals, concept papers and other materials to obtain funding.  The qualifications for this position are an associate degree and two years of healthcare, public health or related experience.  A bachelor’s degree in a related field and experience with youth substance use prevention and/or tobacco use prevention is preferred.  This position is scheduled to close 1/3/2021, or until an adequate pool is reached.  This hiring range for this position is $17.80 to $25.21.  New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.

Substance Use Prevention Coordinator

San Juan Basin Public Health is hiring for a Communicable Disease Specialist II. This person will assist with the planning, operating or coordinating of Points of Distribution (PODs) for communicable disease testing and flu/COVID-19 vaccination in cooperation with local healthcare partners; prepare, manage and train potential volunteers; obtain, manage and distribute supplies as required; prepare ongoing documentation and reports; assists the ESF8 lead during Emergency Operations Center (EOC) activations; and work collaboratively to resolve problems within established standards and guidelines. Incumbents may act as a lead worker assigning and reviewing work activities, providing input into performance evaluation and acting as a resource for other staff.  This position is scheduled to close 11/29/2020, or until an adequate pool is reached.  The hiring range for this position is $19.24 to $23.57 an hour.  New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.

Communicable Disease Specialist II

San Juan Basin Public Health is hiring a Communicable Disease Specialist I  to serve as Case Investigation/Contact Tracers. These positions will assist the Southwest Epidemiologist(s), perform case and contact investigations, update data tracking systems regularly, provide recommendations to Southwest Regional Epidemiologist(s), and facilitate and participate in public health planning, training, and exercises. Employees will be devoted to developing, sustaining, and improving upon public health surveillance and epidemiological investigation systems and processes at the local level in Colorado, as well as providing support in the development of a collaborative statewide network providing emergency preparedness and response assistance. Qualifications for these positions are an Associate’s degree and two years of healthcare, public health or related experience.  Bachelor’s degree is preferred. Communicable Disease Specialist I

Job Description

San Juan Basin Public Health is hiring a COVID-19 Liaison Officer to act as a lead for partnerships with agencies and community organizations responding to the COVID-19 pandemic. The position will establish liaison counterparts at partner agencies, relay appropriate information to partner agencies on behalf of SJBPH, relay appropriate information and requests back to SJBPH on behalf of partners and identify appropriate collaborative solutions to identified issues and problems. The position will also coordinate the regional incident policy group, containing elected officials, agency administrators, and representatives from school districts, state and federal agencies, and tribes. The Liaison Officer will also attend community meetings and track developments in public health statutes, orders, advisories, and technical guidance, and will serve as Deputy Clerk to the San Juan Basin Board of Health.  This position is scheduled to close 9/4/2020, or until an adequate pool is reached.  The hiring range for this position is $33.34 to $41.68. New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description.
